Familial cold-induced urticaria

(FCAS acronym for Familial Cold-induced Auto-inflammatory Syndrome)
Hereditary periodic fever due to a mutation of the CIAS1 gene (Cold Induced Auto-inflammatory syndrome 1) coding for cryopyrin (expressed in monocytes, neutrophils, and chondrocytes.), also known as NALP3, PYPAF1, or NLRP3, a regulating factor of the production of several cytokines . The condition is characterized by the occurrence of a febrile attack with skin rash, conjunctivitis, and arthralgia following exposure to cold.
Anesthetic implications:
as for chronic urticaria, it is wise to premedicate with H1 antihistaminics and avoid COX1 NSAID's, except if it is sure that the patient usually tolerates them well.
