Tolosa-Hunt syndrome

Painful ophthalmoplegia affecting all age groups, and characterized by:

-        acute attacks (for a period ranging from a few days to a few weeks) of periorbital pain.

-        paralysis of the homolateral oculomotor nerve

-        ptosis

-        uncoordinated ocular movements

-        visual impairment

This clinical picture is usually secondary to an unspecific inflammatory process  of the cavernous sinus or of the superior orbital fissure.

Evolution is unpredictable with spontaneous remissions or recurrent crises

Anesthetic implications:

exclude a thrombosis of the cavernous sinus ? often, high doses of corticosteroids
