Cardiac shunts

Description of different shunts used in pediatric cardiac surgery and their usual indications.

Definition

Indications

Possible implications

Blalock-
Taussig

termino-lateral anastomosis (sometimes latero-lateral) between the subclavian artery and a branch of the PA

Tetralogy of Fallot, pulmonary atresia, tricuspid atresia

Disappearance of the pulse on the side of the shunt

Waterston

Anastomosis of the right PA with the descending Ao (rare)

Tetralogy of Fallot, pulmonary atresia, tricuspid atresia

Potts

Anastomosis left PA with the descending Ao (rare)

Tetralogy of Fallot, pulmonary atresia, tricuspid atresia

Glenn

termino-lateral anastomosis of the SVC with the right PA which is disconnected from the left PA

Tetralogy of Fallot, pulmonary atresia, tricuspid atresia

Ebstein anomaly

Risk of superior caval syndrome and intracranial hypertension if pulmonary resistance is high

Modified Glenn

(bidirectional)

termino - lateral anastomosis of the SVC with the right PA that is still connected to the left PA

Tetralogy of Fallot, pulmonary atresia, tricuspid atresia,

Ebstein anomaly

Fontan

termino-lateral anastomosis of the SVC with the right branch of the PA

+

termino-lateral anastomosis of the IVC on the PA (with a conduit in or near the RA)

Pulmonary atresia,
tricuspid atresia

LV hypoplasia

Risk of portal hypertension (ascites), flutter,
venous thrombosis.

Requires a good function of the single ventricle
and a sinusal rhythm

Central shunt

Goretex shunt  between the ascending Ao and the common trunk of the PA

All cyanotic heart disease

Pulmonary artery banding

("banding")

Reduction in flow in the PA
by placing a U lace, tightened depending on hemodynamic response of the child

All heart diseases with pulmonary overflow to avoid the development of an irreversible PAH (VSD,  atrioventricular canal, some cases of LV hypoplasia instead of Norwood I): it aims at reducing the  pulmonary pressure beyond up to 30-50 % of the aortic pressure

Pulmonary flow resistance increases with time: risk of reversal of the shunt
in case of
a systemic pressure drop

Rashkind procedure

interatrial septostomy  by catheterization (balloon catheter) in the newborn

Transposition
of the great vessels

Blalock-Hanlon procedure

surgical interatrial septostomy (> 3 months)

Transposition
of the great vessels

Sano

Conduit connecting the R ventricle to the PA junction

LV hypoplasia, alternative to Norwood 1 procedure
