(Hepatoadrenal syndrome)

Insuffisance surrénalienne relative associée à une cirrhose sévère (30 %  à 70 % des cas).

Elle peut ne se révéler qu’à l’occasion d’un sepsis ou d’une hypovolémie (hémorragie), ou après une transplantation hépatique.

La physiopathologie est inconnue : anomalie du métabolisme du cholestérol, inflammation, hémodynamique ?

Signes cliniques : hyponatrémie, hypoglycémie, hypotension artérielle.

Diagnostic : cortisol plasmatique bas ou à la limite inférieure de la normale (<7 µg/dL) et ou  une réponse insuffisante à un test à l’ACTH (taux maximum < 18 µg/dL ou augmentation < 9µg/dL à 30 min). Il semble que le taux de cortisol salivaire soit plus fiable que le taux sanguin (influence de l’hypoprotéinémie).

Diagnostic différentiel : effet des diurétiques, insuffisance surrénalienne secondaire à un traitement à base de cortisone ou à un infarctus des glandes surrénales (choc hémorragique, ligature artère surrénale droite durant la phase de dissection d’une  transplantation hépatique)

Implications anesthésiques: 

y penser en cas d’hyponatrémie et d’hypoglycémie, ou de réponse insuffisante à la noradrénaline pour traiter une vasoplégie.
