(diprosopus)

Rarissime. Les filles sont deux fois plus affectées que les garçons.

Duplication

• soit de toute la face : deux visages sur un même crâne ; parfois associée à une anencéphalie, une duplication du système nerveux central ou un spina bifida
• soit d’une partie de la face. Par exemple, deux bouches dont l’une est en position normale et l’autre est ectopique (photo) ; dans ces cas, les cavités buccales communiquent ou non et les mouvements de succion sont synchrones. La duplication peut inclure la mandibule, le maxillaire, une partie du palais. La cause en est probablement la duplication du premier arc branchial. Il y a parfois des anomalies cérébrales associées notamment une duplication de l’hypophyse et/ou de l’hypothalamus. Une fente de Tessier (notamment de type 7) (voir ce terme) peut être associée.

Ces duplications font souvent partie d’un syndrome et sont alors associées à un syndrome de Klippel-Feil, une triade de Pierre Robin ou une fente labiopalatine.

Implications anesthésiques: 

prévoir une intubation/ventilation difficile (application du masque facial) ; une intubation sous-mentale peut être nécessaire pour faciliter l’abord chirurgical complexe
